The 31-year-old woman was last seen July 11, driving a 2007, silver Jeep Liberty with the license place HAPPIRN (Happy Nurse).

Woolard has brown hair and brown eyes and is 5 feet, seven inches and 220 pounds.

She has a scar on the left side of her face and a tattoo on her lower back of a three-leaf clover and a frog on her left shoulder.
